PASSPORT INFORMATION
The following is information that has been compiled to assist you in the
application
process of applying and/or renewing a passport. Application forms are available
on-line at http://travel.state.gov/
or in the Clerk of Courts office.
ROUTINE SERVICE – Customers will receive passports within 8
weeks from the date of application.
EXPEDITED SERVICE – Customers wishing to receive passports
within 2 weeks may request expedited service. Add $60 per application
and 2-way
overnight delivery. ***Expedite fee does not include overnight delivery***
FIRST TIME APPLICANT
(FORM DS-11)
Items needed to process a passport:
1. Certified copy of birth certificate, naturalization papers, or old passport
*A child under the age of 14 who had a previous passport must submit BOTH
passport & certified birth certificate
2. Driver’s License(Child under age 18 a parent’s driver’s license will be used)
3. 2 – 2” x 2” passport photos (Walgreen’s, Wal-Mart, Kinkos, Sears or any
photographer)
4. Check or money order payable to US DEPARTMENT OF STATE
$75-reg. adult passport $60-reg. child passport(age 0-15)
$135-expedited adult passport $120-expedited child passport(age 0-15)
5. Processing fee to Clerk of Courts for $25 (can be cash or check)
PASSPORT RENEWAL
(FORM DS-82) – Customer submits
application to passport agency directly:
You may use this form if your previous passport:
1. Was issued when you were 16 or older
2. Was issued in the last 15 years
3. Is not damaged
4. Your name is the same
5. Is submitted with your application

SPECIAL REQUIREMENTS FOR MINORS UNDER AGE 14
*** MINOR MUST APPEAR IN PERSON***
1. Both parents must appear together and sign or
2. One parent appears, signs and submits second parent’s NOTARIZED statement of
consent authorizing the issuance of the passport for the minor child (a
notarized
Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor
Under Age 14), or a notarized written statement with the
same information on a sheet of paper from the non-appearing parent that includes
the child’s name and date of birth, as well as parent’s identification
information or a copy of his/her ID may be used for this purpose or
3. One parent appears, signs and submits primary evidence of sole authority to
apply (such as one of the following):
Child’s certified US or foreign birth certificate (with
translation, if necessary) listing only applying parent; or
Consular Report of Birth Abroad (Form FS-240) or
Certification of Birth Abroad (Form DS-1350) listing only applying parent;
or
Court order granting sole custody to the applying parent
(unless child’s travel is restricted by that order); or
Adoption decree (if applying parents is sole adopting
parent); or
Court order specifically permitting applying parents or
guardian’s travel with the child; or
Judicial declaration of incompetence of non-applying
parent; or
Death certificate of non-applying parent.
If none of the above documentation is available, the applying parent/guardian
should submit a
Form DS-3053: Statement of Consent: Issuance of a Passport to
a
Minor Under Age 14.
NOTE:
A third-party in loco parentis applying on behalf of a minor under the
age of 14 must submit a notarized written statement or affidavit from both
parents or guardians authorizing a third-party to apply for a passport. When the
statement or affidavit is from only one parent/guardian, the third-party must
present evidence of sole custody of the authorizing parent/guardian.
