Health and Human Services Aging Services Unit1. PROGRAM: ELDERLY NUTRITION PROGRAMDescription: This federal program provides midday congregate and home delivered meal services to persons 60 years or older on a donation basis. The Elderly Nutrition Program utilizes more than 150 volunteers countywide to transport home delivered meals and serve meals at congregate meal sites.a. SUB-PROGRAM: CONGREGATE MEAL SERVICECongregate meal service provides nutritious meals and fellowship in a communal setting. Congregate meal sites reduce isolation, offer recreational activities, health screenings, diet and benefit counseling, tax and energy assistance, volunteer opportunities, peer support, educational programming and current informational updates affecting older persons.County-wide congregate meal sites are located at the following accessible locations: Greenville Safety Building, Golden Venture Apartments - Kaukauna, Oneida Heights Apartments - Appleton, Randall Court Apartments - Appleton, Good Shepherd Complex - Seymour, and Freedom Town Hall.b. SUB-PROGRAM: HOME DELIVERED MEAL SERVICEHome delivered meal service aids homebound older persons who are unable to prepare meals for themselves, unable to attend a congregate meal site and have no other resources to assist them with meal provision. In addition to providing a nutritious meal, home delivered meal service provides an opportunity to observe and insure recipient well-being and safety.Eligibility: Persons age 60 years and older may eat at a congregate meal site or receive a home delivered meal. Home delivered meal recipients must reside within Outagamie County and are assessed for meal service.Fee: No fee. SUB-PROGRAM: VALLEY TRANSIT IIDescription: Aging Services contracts with Valley Transit to provide specialized urban transportation for Outagamie County persons with a disability and persons 60 years and older residing within the Fox Cities (Appleton, Grand Chute, Little Chute, Kimberly, Combined Locks, and Kaukauna). This specialized transportation, called Valley Transit II, is provided by Kobussen Buses, Ltd. Riders much be certified to travel within the urban system.b. SUB-PROGRAM: RURAL TRANSPORTATIONDescription: Aging Services contracts with Kobussen Buses, Ltd. to provide specialized rural transportation for Outagamie County persons with a disability and persons 60 years and older residing outside of the Fox Cities.c. SUB-PROGRAM: NEW LONDON SENIOR AND DISABLED TRANSPORTATIONDescription: Aging Services contracts with the City of New London to provide transportation to Outagamie County persons with a disability and persons 60 years and older residing within an 8 mile radius of New London.Eligibility: Outagamie County residents with a disability and/or 60 years and older.Fee: Subsidized fares applicable to transportation system(s) utilized.For Further Information, Contact: Aging & Disability Resource Center at (920) 832-5178, Valley Transit: (920) 832-5800, New London: (920) 982-8532, Kobussen Buses, Ltd.: (920) 766-2247 or 1-800-447-01163. PROGRAM: HOME HANDY PERSONDescription: The Home Handy Person Program provides minor home repairs to Outagamie County homeowners 60 years and older. Home Handy Person program requests include basic electrical, plumbing and carpentry repairs. This excludes cleaning, sanding, stripping, painting, storm window and screen installation, cosmetic home remodeling, plus any repairs which require a building, electrical or plumbing permit. There is no cost to participants for labor, however, participants are responsible for payment of supplies and are encouraged to make a donation.Eligibility: Outagamie County homeowners 60 years and older.Fee: Parts and supplies needed are paid for by the participant. PROGRAM: SENIOR SHOWCASEDescription: Senior Showcase is a weekly television show produced by older volunteers. Volunteers perform production duties such as selecting Senior Showcase topics, program scheduling, interviewing guests, graphics, taping, plus other technical duties. Senior Showcase airs Thursdays at 8:30 p.m. on Time Warner Cable Channel 2 and 4.Eligibility: Anyone interested.Fee: No feeFor Further Information, Contact: Aging and Disability Resource Center at (920) 832-51785. PROGRAM: TEMPORARY RESPITE CAREDescription: Temporary Respite Care is contracted through Lutheran Social Services. The program provides a brief period of relief or rest for caregivers by dependable volunteers. The program recruits, trains, and professionally supervises volunteers. The program goal is to enable caregivers to be temporarily relieved from care giving on an intermittent, occasional or emergency basis.Eligibility: Primary caregivers who are caring for dependent persons 60 years and older or grandparents 60 years and older caring for minor grandchildren.Fee: No fee. PROGRAM: NURSE ADVOCATE/WALK IN HEALTH CLINICDescription: The Nurse Advocate/Walk In Health Clinic is contracted through the Thompson Community Center. It is staffed by a registered nurse providing the following services on Tuesday and Thursday mornings: blood pressure or sugar screening, nutrition and medication counseling, and general health assessment. By appointment, toenail care and ear irrigation are provided.Eligibility: 60 years and older.Fee: No fee. PROGRAM: MENTAL HEALTH COUNSELINGDescription: Mental Health counseling is contracted through Lutheran Social Services. Masters level counselors provide direct counseling to older persons and their families on an outreach or in-house basis. Older persons are encouraged to access counseling at the Thompson Community Center, but counselors do go to the older person's home as needed. Common issues include adjustment to change and loss, grief, major health changes, depression, anxiety, etc.Eligibility: 60 years and older.Fee: No fee. PROGRAM: “SIDE BY SIDE” AODA DAY TREATMENTDescription: “Side By Side” is a state certified, intensive outpatient, alcohol and drug abuse program. It is designed specifically to meet the age-specific needs of older persons and family members. Services include individual, family and group counseling; AODA education; social and recreational involvement; and health clinic participation.Eligibility: 60 years and older.Fee: A suggested fee has been established, however, services are not withheld if a participant has a limited ability to pay the suggested fee.For Further Information, Contact: Thompson Community Center at (920) 733-28609. PROGRAM: INFORMATION AND ASSISTANCEDescription: Information and Assistance is contracted through Lutheran Social Services and is provided through Senior Connection (located at the Aging and Disability Resource Center). Information and Assistance is a service that informs participants of the opportunities and services available to older persons, assesses the problems and capabilities of participants, links participants to the opportunities and services available, ensures that participants receive the services needed through follow up procedures, and advocates on behalf of participants when needed services are not being adequately provided.Eligibility: Family caregivers and persons 60 years and older.Fee: No fee. PROGRAM: CAREGIVER EDUCATION AND SUPPORTDescription: Caregiver Education and Support is contracted through Lutheran Social Services and is available at the Thompson Community Center. A training program will be offered to caregivers bay master trainers to provide caregivers with the tools to increase self-care and the ability to handle difficult situations, emotions and decisions.Eligibility: Family caregivers of any age caring for persons 60 years and older.Fee: No fee.
