WHY IMMUNIZE?
Many diseases can be
prevented by immunizations. Wisconsin’s immunization law requires
children in licensed day care centers and students through grade 12 to
be immunized against certain diseases according to their age or grade
level. Students must provide dates of immunizations upon admission to
school. It is important to keep an up to date record for everyone in
your family. Please bring immunization records with you whenever you
visit your doctor or immunization clinic to assure that the proper
immunizations are given.

WHAT IS WIR?
Wisconsin Immunization
Registry (WIR) is an electronic database that was developed to keep
track of children’s immunization records and help them follow the
recommended schedule. It helps providers remind parents when
immunizations are due. The Public Health Division uses the WIR to
record immunizations given, to assist parents and medical providers
determine immunization status, and to track immunization rates for the
community. Parents may request to opt out of the registry.
