COPIES OF VITAL RECORDS

IN PERSON APPLICATIONS

You can apply in person for a copy of a birth, death or marriage certificate in the Register of Deeds Office in the County where the event actually occurred.  

For Outagamie County, the office is located at 410 S Walnut St, Appleton on the main floor of the County Administration Building.  Office hours are 8:00 am to 4:30 Monday-Friday.  Summer hours from Memorial Day to Labor Day are 7:00 am to 3:30 pm Monday-Friday.  Acceptable payments for in-person requests are cash, personal check or money order.  We do accept debit and credit cards (additional fee required.)

In order to receive a certified copy, you need a direct and tangible interest as defined on the application form.   Application forms are available in our office or you may fill out the application on this website and bring it along with you. 

Identification is required to pick up copies of vital records in person.  The acceptable forms of identification are:  Wisconsin driver’s license or Wisconsin ID with photo, Out-of-State driver’s license/ID with photo.  All forms of identification must show your current name and address.  Expired licenses will not be accepted.

We do not accept phone orders or verify any vital record information over the phone.

A photocopy of the applicant's current valid photo ID must accompany all mail applications for birth certificates.

All Requests for copies received through the mail are processed the day the request is received.  Please include a business sized self addressed stamped envelope along with your completed application, identification and the correct fee.
